Can anyone recommend a good proxy host which has high uptime, reliability and bandwidth - and doesn't cost too much?

I've tried so many, but inevitably each fails

Have you tried dreamhost?

In their terms of service it says that proxy websites are not allowed on their servers:

I've recently heard about one called cloakhost

It has fairly good reviews, so if i ever start a proxy i will consider using it.
